Our receptionists will ask you to complete registration forms (if you haven’t already) including your address, phone numbers, contact person and insurance coverage.
You will need the name and address of the referring physician or optometrist as well as a general medical information form. Our staff can assist you in completing these forms if necessary, and they will provide you with our health care privacy policy.

A certified ophthalmic technician will then take an initial history and obtain preliminary measurements, followed by a thorough medical exam by our physicians. Drops may be used to dilate your pupil to permit a complete eye exam. The drops usually take 20-30 minutes to work. Please note that this pupil dilation may temporarily interfere with your ability to drive. If this is a concern, please make arrangements for someone to drive you home following your visit.

In preparation for your exam, please compile the following items and bring them to your appointment: 

1. Latest glasses
2. Referral forms
3. Insurance card(s)

Please be sure to take all of your regular medications, including eye drops, at their normally scheduled times. It may also be helpful to bring a driver if you are concerned about dilation after your exam.

Diagnostic testing will be performed during your appointment. An initial exam is very complex
and usually takes a few hours.

Please bring ALL eye medication bottles
A list of ALL medications
A letter from your referring doctor including any eye diagnoses and any eye surgeries

We do not have an optical shop and would recommend that you see your referring optometrist or ophthalmologist for glasses. We do not routinely do refractions for glasses, but will do so on request. PLEASE NOTE SINCE WE ARE A MEDICAL PRACTICE AND DO NOT ACCEPT VISION INSURANCES THERE WILL BE A $40.00 FEE FOR ANY REFRACTION PERFORMED.

We accept most major insurances and our staff will be happy to confirm that we accept your particular plan prior to your visit. Please remember to bring your referral form in you are an HMO member. It is your ultimate responsibility to meet any co-pays or deductible. We will gladly file your claims and our billing specialists will be happy to answer any of your questions. Please note when you schedule an appointment please have your insurance information available to provide to our staff.
